Care Coordinator

Job Title:  Care Coordinator


Department / Location: Stockport – based in specific branch office with visits to carer &

customers as required


Reports to: Registered Manager


Responsibilities: To allocate the right cares to the right customer at the right time.

Salary banding: competitive


Hours: 37.5 hours a week - 5 days out of 7-day( including weekends) contract flexible to

meet the needs of the Business


 Salary:- depending on qualifications and experience. All job offers are subject to enhanced DBS clearance and satisfactory references. 


 An excellent opportunity has now arisen for a Care Co-ordinator to join our home care team based in our Wythenshawe office, on a permanent full time basis 


 This role requires someone who has already had experience of working in a fast paced homecare business and is used to juggling lots of tasks and delivering great customer service to customers and colleagues. Within this role you be responsible for managing the rotas of the care staff within this busy domiciliary care business, maintaining electronic records and supporting on any incoming queries .


Previous experience as a Care Co-ordinator is essential. Experience of Rota Management in domiciliary and Electronic Call Monitoring Systems is essential, as you will be required to undertake Co-ordinating. NVQ 3 (or above), in Health and Social Care, essential.


 To apply for the Care Coordinator role you will need : - 


  •  Have minimum of 2 year's experience in care scheduling or rota Management gained from within the care sector ·
  •  Be able to efficiently manage rotas and schedules for both clients and carers · 
  •  Act responsively to schedule changes including sickness and holidays · 
  •  Develop strong working relationships with all stakeholders · 
  •  Ensure that staffing levels are appropriate to client demand ·
  •  Have a positive mindset and enjoys problem-solving .
  •  Upload data to and accurately maintain an electronic scheduling system 


Primary Responsibilities:


  •  To allocate permanent calls / runs to cares ensuring continuity
  •  To ensure that immediate calls are covered.
  •  Communicate with customers when there are changes to calls.
  •  Communicate with cares when there are changes to their work at short notice.
  •  To plan and organize future cover up to generation and to ensure all cares allocated

                  work is given to them in a timely manner

  •  Complete Care Calls if no other carer can be provided
  •  To ensure that the permanent is at 3% or under within your postcode areas.
  •  To take accountabilities for all conversions for new packages on Brokerage Team.
  •  Take responsibility for decision making in regard to new packages to enable the

                  business to grow.

  •  To plan and organize interview, shadowing & coaching for new recruits.
  •  To conduct interviews and orientation for new recruits.
  •  To conduct Supervisions and Direct Observations every 3 months or as required.
  •  To support the Registered Manager with customer visits and reviews as and when

                  required.

  •  Undertake New Business assessments and produce individual care and support        plans and risk assessments for new customers if required.
  •  Liaise with Recruitment to ensure they are aware of staffing requirements
  •  All carer and customers user records to be kept live and accurate.   
  •  To support other postcode areas with Carer’s during holidays / absence
  •  Cover Out of Hours Service on a Rota Basis
  •  Support Registered Manager with Carer Recruitment
  •  Be confident and capable to run the branch in the managers absence
  •  Any other duties required by the needs of the business.

Location: Our working locations are  Heald Green, Gately, Edgeley and Cheadle. 


Working hours : Full time / Part time. 


 Shift details: Long days, Half days (07.00 to 14.00 or 15.00 to 22.00)


Job Description and Job Specification Purpose of roleTo support service users with all aspects of their day to day living, so they can enjoy the best possible quality of life. Providing care and support is both a challenging and rewarding experience. You will mostly work alone with the service user in their home. Compassion, good communication skills, and a calm and caring manner are essential for this important role in our company.


 Key responsibility : Safely provide compassionate care and support that is centered on the individual needs and wishes of each service user. Respect service users’ choices and always promote their dignity. You may be the only person the service user sees over a period of time; it is therefore essential to report any changes or causes for concern to your line manager promptly. You need to be clear about when to seek help and advice in order to keep service users safe and promote their well-being. 


Duties Care and support Give non-discriminatory care and support that values the diverse and unique qualities of each service user. See the whole person and not merely a list of care needs. Carefully listen and observe how service users prefer their care and support to be delivered on a day to day basis. Help them make their own decisions and to be as independent as possible. Follow instructions in the care and support plan which has been agreed with each service user. This may include:

  • All aspects of personal care
    • Showering and bathing
    • Dressing  and grooming
    • Toileting  and continence care
    • Teeth and dentures
  • Taking medicines
    • Encouraging,  reminding; assisting and giving medicines
    • Ordering  and collecting prescriptions
    • Returning unwanted medicines to the pharmacy for safe disposal
  • Eating and drinking
    • Helping the service user to plan what to eat and drink
    • Gentle encouragement and help to eat and drink well
    • Shopping, preparing and serving food and drinks
    • Clearing   the table, washing up and keeping the kitchen area clean and tidy;
    • Agreeing  with the service user how to store food safely and dispose of out of date       produce
  • Safely using aids and personal equipment in a manner that respects the dignity of service users. For example
    • Standing  and walking frames
    • Wheelchairs, manual and electric hoists
    • Sliding sheets and moving boards
    • Hearing  aids and other physical aids
  • Housework
    • Washing  floors, vacuuming and sweeping
    • Laundry and   ironing, making beds and changing the linen
    • Dusting and general tidying
  • Social and physical activities or      mental stimulation such as
    • Answering  the door and greeting visitors
    • Answering emergency bells and the telephone
    • Writing cards and letters or emails
    • Taking a  service user out shopping, to see their friends or to other activities
    • Hobbies and  recreations such as reading, photo albums, games, etc.
  • Supporting a service user through temporary and terminal illness, including
    • End of life  care
    • Hospital   appointments
    • Liaising  with community health support and families   Recording and reporting
  • Record and report all relevant      service user information including
    • The care       and support that you provide and assistance with medicines
    • Changes to       a service user’s condition or other concerns
    • Faulty       equipment or hazards in the home
    • Response to       emergencies, accidents and incidents
    • Safeguarding       matters
    • Contact       with families or Carers and other professionals
    • Other       matters as required by Angel Homecare Service procedures
  • Keep all information about      service users and their families secure and confidential   Work well as part of the Angel Homecare Service team
  • Follow Angel Homecare Service policies, procedures and guidance at all times
  • Take part in staff and service User meetings
  • Attend training activities and  appraisal and development meetings

This list is not exhaustive and from time to time you may be required to undertake additional duties. We will provide full training in line with regulatory requirement.


 

Applicants should meet the following criteria:

  • You will have previous experience as a Care Assistant (6 Months+)
  • Have a passion and genuine desire to provide excellent care to residents and patients
  • All applicants must have the right to work in the UK
  • You will have strong communication and organisational skills an
  • You will be reliable and punctual, trustworthy and honest
  • Be willing to undergo a Disclosure and Barring Service check.
